I am excited to inform our community and health care partners that the Erie St. Clair LHIN advised us on October 6, 2011 that in addition to the funding support for the current fiscal year for the 10 Assess/Restore beds, the Hospital will be receiving funding in the amount of $1,168,000 for each of the next two fiscal years to allow us to continue the operations of the Assess/Restore program. 

 

The program is designed to provide additional therapies and support to allow patients to return home and avoid future readmissions to an acute care bed.  By providing additional therapies and nursing support along with pharmacy support patients can avoid the designation alternative level of care (ALC) and prolong waits in the hospital for a long term care bed.  The program has been instrumental in decreasing long-term care home wait list in the Windsor-Essex area.

 

We will continue to work with our community partners and Community Care Access Center (CCAC) and report our results on a quarterly basis to the Erie St. Clair LHIN.
